This is a sad case but I find it not really germane that the man was transed.

Gardacil vaccine has been available in the US only since 2006, and as of around 2010 it was not marketed except for women, and it was intentionally not marketed to men yet.

It was specifically not marketed to gay men who, had epidemic levels of anal cancer due to HPV - there was fear that the marketing would confuse women (I went to conferences on the subject; several of my gay male friends died of anal cancer in the late 2010’s). I did not see general advertisement of HPV vaccination until after 2020 in San Francisco, gaytown central.

To date men, most gay men are still not aware of several basic health measures including Hep-B and HPV vaccination. HPV virus infection which can progress to cancer is not the HPV which causes visible warts - there are dozens of HPV variants.

Anal swabs (Tush Smear similar to a Pap smear) for neoplasms are still not part of regular check-ups unless asked for. I ask all my gay friends to get regular tush smears, and to get vaccinated. Many were not aware of the existence of cheap, simple testing.

These cancers are extremely slow-growing. Unless you are swabbing, by the time you have detection it is not unusual to be at stage III - metastasized.

A man with a radical surgery such as this has the same chance as any man to develop HPV related cancer; to allow it to progress slowly, unnoticed. The main elevated risks would be damage to nerve tissue leaving less sensation in the area of a tumor; the pouch as an open wound would not be surprising to have a discharge; and lack of a cervix would make whichever doctor less likely to perform any swabbing and testing for neoplasms.
