Ruby is now in hospital fighting sepsis after gender-affirming male-to-female top surgery.

Sepsis related brain injury symptoms can look like a lot of things, including attention issues, emotional regulation problems, depression, and behavior/judgment issues.

PTSD from this severe medical trauma can look similar to a brain injury and it can be hard to tease these two independent causes of functional impairment apart. Brain damage can also be very focal, like losing a previously learned skill such as speaking. If I were in Ruby's position I would want some sort of scan, preferably MRI, to see whether there were any significant brain lesions during this episode, which may be located in areas controlling my breathing reflex, resulting in things like central sleep apnea, which can cause a lot of problems and may require further treatment. I would also want SLP evaluation to establish cognitive baselines. Please be kind to yourself in the coming weeks and months. Sepsis is a profound and complex injury.
